Clinical Social Workers

Having clinical social workers alongside health care providers is a new model being used at Franklin Health primary care medical practices. Social workers perform behavioral health services on the spot, as well as by primary doctor referral, providing assessment and diagnosis, and brief counseling for patients of all ages. Your doctor or nurse may ask you to take a moment to meet your practice’s social worker. They look forward to working with you!
