No matter what care your patients need, MaineHealth Care at Home can help across our home care, palliative care, and hospice programs. MaineHealth Care at Home Referral Center has extended hours to serve you better. Referral Specialists can answer questions, arrange services, and clarify insurance coverage. A liaison nurse may be assigned to work directly with a patient's hospital, rehabilitation facility, nursing home, case manager, or physician to smooth the transition home.

Upon request, we can arrange in-services to professional staff on topics to include:
- Home health benefits and services
- Eligibility and reimbursement programs
- Care topics to include wound care, telehealth, diabetes, rehabilitation programs, post-operative joint replacement services, and palliative care.
In addition, our Provider Relations Team works with the healthcare community to facilitate services that ensure a smooth transition home from a hospital or skilled care facility. We also have assigned liaisons to community hospitals to help facilitate referral and care transitions.

On January 1, 2015, the Centers for Medicare & Medicaid Services (CMS) made additional changes to the Face-to-Face (F2F) documentation requirements that are part of the eligibility standards for all Medicare home health agencies. When referring to home health, the certifying physician must include documentation that substantiates:
- Need for skilled services
- Homebound status
- Services under a physician plan of care
- Face-to-Face encounter occurred 90 days prior to or 30 days within the home health start of care

MaineHealth Care at Home maintains a comprehensive program for outcome-based quality improvement. Our efforts include utilization of a comprehensive benchmarking service though Strategic Healthcare Programs.
In addition to these efforts, we contract with a national consulting firm to survey our patient satisfaction.
