Request and manage appointments
Request an appointment anytime
With MyChart, you can request an appointment online, even when your doctor's office isn't open. You'll even get an email reminder a few days before each appointment. You can also view past appointments, cancel appointments and get appointment related information like directions, telephone number and instructions any time of the day.
To request an appointment, after logging into your MyChart account, point to the Appointments option and click on Request an Appointment.
See history of your appointments
To view past appointments, after you log into your MyChart account, point to the Appointments option in the navigation bar and click on Past Appointments in the drop-down menu. MyChart will display a list of previous appointments by date.
By clicking on each date, you will see a general summary of that appointment, including the reason for the visit, your vital signs at the time of the visit and any follow-up instructions.
Cancel appointments online
Appointments that are scheduled to occur in more than 24 hours can be canceled online. However, appointments that will occur in less than 24 hours cannot be canceled online. Please contact your doctor's office directly to cancel and/or change these appointments.
With MyChart, all your current medications are neatly listed in one place. Forgot the dosage of a new medicine? Look it up and request prescription renewals from your doctor all from the convenience of your home or mobile device. If you have refills remaining, you can simply call your pharmacy and request a refill.
However, if you have no refills remaining, renewing prescriptions is easy and fast. When your doctor approves your refill request, pick up your script at your favorite pharmacy.
After logging into your MyChart account, point to the Medications option, then click on Request a Renewal option and fill in the requested information.
Send secure messages to your provider
No more playing phone tag when you need to share information with your doctor. If you have a simple question about a medication or test result, simply send a secure message to your doctor in MyChart.
In the case of an emergency, do not use MyChart to contact your doctor. Please contact your doctor directly by telephone or call 911.
After logging into your MyChart account, point to the Message Center option and click on Contact Your Doctor. View the following image as an example.
View test results
Get your lab test results as soon as they're available and track how you're doing over time. With MyChart, you'll see an explanation of your results, and when appropriate, see them compared to results that are considered normal.
Care for your loved ones
With MyChart, you can securely access medical information about your child, your aging parent or someone in your care.
In order to gain proxy access for your children or aging parent, you must make this request in person upon checking-in for one of their outpatient visits. If the child is under the age of 13 the staff will be able to grant you proxy access. However, if the child is 13 years of age (or older) you and your child will need to complete the release of information form provided by the front desk staff. Once you have completed this form, the staff will be able to grant you proxy access to their records using MyChart. Similarly, adults wishing to grant proxy access to other adults will need to complete the release of information form before the proxy access can be granted.
