The MaineHealth ACO has released a short new video that introduces viewers to a simple rubric for holding successful code status discussions. The video offers a quick primer on the rubric and shows how a provider can use it to help a patient make an informed, confident decision about end-of-life care. The ACO produced the video in collaboration with Dr. Rebecca Kowaloff, a palliative care physician at Southern Maine Health Care, and the MaineHealth Educational Services video production team. The video, along with a downloadable pocket guide, can be found at mainehealth.org/mainehealth-accountable-care-organization/provider-resources/code-status-discussions.
For in-depth guidance on how to have effective goals-of-care conversations with patients, we recommend participation in the Serious Illness Care Program. Please contact Deb Silberstein for more information.
