We are a Level Three Patient-Centered Medical Home offering the highest quality care. Our care team includes family medicine providers, internal medicine providers, pediatricians nurse practitioners and non-clinical support staff. We are here to answer your health questions, provide preventive care, order tests, prescribe medications, help you manage chronic conditions and access specialty care when needed.

Primary care providers (PCPs) have a broad range of expertise and play an important role in your everyday health care. Except for emergencies that require urgent medical attention, your PCP should be the first person you contact about a health concern. Your PCP can:
- Take care of routine medical problems such as aches, pains and colds
- Provide annual check-ups and other preventive care
- Diagnose and monitor chronic conditions
- Provide health education and prescriptions
- Refer you or your child to a specialist when needed
Choose a primary care provider who is the best fit for you and your family:
- Family medicine doctors have special training to provide medical care to patients of all ages. Family doctors can care for your entire family.
- Pediatricians are specially-trained to diagnose and treat children. Pediatricians also provide well-child visits.
- Internal medicine doctors, also known as internists, diagnose and provide treatment for adults.
- Family nurse practitioners are registered nurses with specialized training to deliver care that is focused on the family. They diagnose illnesses, preform exams and prescribe medicine. They work in primary and specialty care practices, such as cancer care, heart care or pain management.
The patient-centered medical home (PCMH) is a health care delivery model of care that puts patients at the forefront. PCMHs build better relationships between patients and their clinical care teams. Research shows that PCMHs improve quality and the patient experience, and increase staff satisfaction—while reducing health care costs. Practices that earn recognition have made a commitment to continuous quality improvement and a patient-centered approach to care.
PCMH recognition is obtained through an evaluation process with the National Committee for Quality Assurance (NCQA). The steps in this process are the most widely-used method to transform primary care practices into medical homes. Intensive Outpatient Treatment for Substance Use Disorder (SUD)
Comprehensive addiction medicine provides outpatient substance-use treatment in a safe, secure, confidential and therapeutic environment designed to support recovery. Our team assists you during every step of the recovery journey, providing comprehensive medication management, behavioral health care, counseling and patient support.
Treatment
Our team provides care and works with you to develop individualized treatment plans based on your specific needs so you can continue your life and work. Services include consultations on all substance use disorders including alcohol, opioids, stimulants, cannabis and tobacco.
