It’s important to us that our patients understand their neurological conditions as completely as possible. We strongly encourage you to ask us about anything that concerns you. Below you will find a list of helpful websites along with medical forms you can print and fill out before your appointment.
Medical Forms
To help make the most of the time spent with your physician, please complete your medical forms and bring them with you to your appointment. We usually mail copies of the forms you’ll need to your home before your appointment. Additional copies are available below for your convenience.
- Physician’s Office Consent to Treat: Completed by new patients, and then once each year.
- Patient Registration: Completed by new patients.
- Medical History: Completed by new patients, or if a patient has not been seen at Maine Medical Partners – Neurology for one year or more.
- Adult Return Visit Questionnaire: Completed when a patient has not been seen at Maine Medical Partners – Neurology for more than 30 days, but less than one year.
- Adult Return Visit Questionnaire (MS Patients):Completed by Multiple Sclerosis Center patients if they have not been seen for more than 30 days, but less than one year.
