Target weight is a baseline weight set for a heart failure patient that he or she monitors daily. If the patient’s weight increases above that suddenly, then immediate clinical action may be in order.

In this important webinar, heart failure specialist Dr. Samuel Coffin will introduce you to the clinical rationale and practical aspects of target weight. He’ll discuss consideration in setting target weight, coordinating care, prognosticating and serious illness conversation. And you’ll be introduced MaineHealth’s heart failure toolkit, including the Healing Hearts booklet, heart failure protocols for various care setting and the MaineHealth Advanced Heart Failure Program.
