Supporting underserved communities through 340B savings

MaineHealth is dedicated to ensuring our most rural, underserved communities have access to a full range of health care services. Section 340B of the Public Health Service Act is critical to our organizations’ ability to serve uninsured and low-income patients. 

Congress passed the 340B program in 1992 as a means for health care providers that serve low-income and uninsured patients to “stretch scarce federal resources as far as possible, reaching more eligible patients and providing more comprehensive services.” The federal 340B program requires pharmaceutical manufacturers participating in the Medicaid drug rebate program to sell outpatient drugs at discounted prices to qualified safety net health care facilities that care for low-income, vulnerable populations. In its more than 30-year history, the 340B program has been critical in helping hospitals serve vulnerable communities across the country. 

MaineHealth Section 340B covered entities include MaineHealth Waldo Hospital, MaineHealth Pen Bay Hospital, MaineHealth Lincoln Hospital, MaineHealth Franklin Hospital, MaineHealth Stephens Hospital, MaineHealth Memorial Hospital, MaineHealth Bleeding Disorders Scarborough and MaineHealth Maine Medical Center Portland.

MaineHealth Franklin Hospital 340B Program Benefits

MaineHealth Franklin Hospital offers a number of programs and services that support our vision of working together so our communities are the healthiest in America. These programs include chronic disease management, behavioral health, substance use, and efforts to address social determinants of health in our communities.  

Health Care Services

Behavioral health services for children, adolescents and adults, including:

  • Medically supervised induction and ongoing medication-assisted substance use treatment
  • Intensive outpatient programming (IOP)
  • Counseling, including individual and group therapy    
  • Case management services for those with behavioral health needs
     

Workforce Training

  • Community paramedicine, including back country search and rescue training

Prevention and Education

  • Healthy food pantry for patients and environmental change work to improve access to healthy options – making healthy food the easy choice
  • Restorative practices policies – alternative to suspension from school for youth

MaineHealth Lincoln Hospital 340B Program Benefits

MaineHealth Lincoln Hospital offers a number of health services and prevention programs to support community health and wellness, including management of chronic disease states throughout the community. Programs are incorporated through a range of mobile health services and worksite wellness/health programs including partnerships with employers and community based organizations.  In FY23, the hospital provided $8,167,514 in uncompensated care for the community.

MaineHealth Maine Medical Center Portland 340B Program Benefits

Incorporated in 1864, MaineHealth Maine Medical Center Portland (MHMMC Portland) is the state’s largest medical center, licensed for 700 beds and employing close to 9,000 people. Our care model includes the state’s largest multispecialty medical group, MaineHealth Medical Group, employing more than 625 physicians. In the greater Portland area, this group provides a wide range of primary, specialty and subspecialty services delivered through a network of more than 30 locations.

MHMMC Portland provides more than 20 percent of all the charity care delivered in Maine. It aims to increase the health of Maine populations by modeling healthy behaviors, educating the public, and providing services for all who need them, regardless of socio-economic factors. In FY2023, MHMMC Portland’s Aggregate “Net Community Benefit Investment” totaled $280,711,923.

Health Care Services

  • School-Based Health Clinics
    • MHMMC Portland provides staff and support for clinics in several Portland Public Schools allowing students to access care without missing school or parents having to miss work.
  • Adverse Childhood Experiences (ACEs) Program
    • An interdisciplinary team of leaders develop and implement best practice models for education, prevention, screening and treatment of ACEs and trauma.
  • Palliative Care Education and Research
    • MHMMC Portland is a medical education site for medical students, residents, fellows and other learners. Several academic papers have been published on the health issues of vulnerable populations and investment in the program has increased multifold in FY 2022.
  • CarePartners
    • CarePartners is a “safety net” program designed to provide care for those who cannot afford commercial insurance but are not eligible for government programs. CarePartners provides administrative support to help serve the target population, including comprehensive eligibility assessment, care management, and access to low cost or free pharmaceuticals.
    • MHMMC Portland plays two roles in this MaineHealth-sponsored program: funding and providing free care. In addition to the free care that MMC gives as a provider in the program, we make a financial contribution to CarePartners program operations.
  • Outpatient Clinics   
    • Many MHMMC Portland outpatient clinics serve specific patient populations with services that would otherwise not be available in the community, including a virology clinic that is a statewide resource to physicians caring for patients with HIV/AIDS
    • It also provides education and conducts clinical trials including many that otherwise would not be available in the State. In addition, one afternoon a week, the international clinic provides services to immigrants and refugees from around the world who have settled in Portland.
  • Northern New England Poison Center
    • The NNEPC serves Maine, New Hampshire, and Vermont with 24/7 toll-free telephone consultations with health care professionals and lay persons about toxic substances. MHMMC Portland contributes funding to provide services that are not sufficiently supported by state or federal government, such as poison- and drug-related outreach education for health care professionals and lay persons, poison and drug-related research, surveillance for terrorism, tampering/contamination, unanticipated adverse drug events, food poisoning and other public health emergencies, and support for all-hazards preparedness and response. The in-kind contribution includes partial salaries for clinical toxicologists, nurse-certified specialists in poison information, and other staff, as well as employee benefits and operating overhead.
  • Care Management      
    • MHMMC Portland assists patients at times with lodging, medicine, transportation and medical equipment.
  • Oncology Patient Navigators
    • Specially-trained oncology nurse clinical patient navigators work with patients diagnosed with cancer to ensure they and their family have the information needed to make the most informed and timely decisions about their treatment plan. MHMMC Portland navigators represent all of the major tumor sites: breast, prostate/genitourinary, lower GI (colon/rectal), upper GI (liver/pancreas), GYN/ONC, and thoracic (lung/esophageal). 
    • A neuroscience patient and family liaison works with neuro-oncology (brain tumor) patients. 
    • MHMMC Portland also pays for half of the expense of an American Cancer Society patient navigator who is available to help patients navigate the myriad of community support resources available as part of the cancer care process.
  • Language Line and Interpretive Services
    • Live and telephonic interpretation services are provided for patients who do not speak English.
  • Sign Language Interpreting Services
    • MHMMC Portland provides on-site interpreters for deaf patients who use American Sign Language and offers accommodations and services for deaf and hard-of-hearing patients and family members. The coordinator trains MHMMC Portland staff to better accommodate deaf and hard-of-hearing patients and, with local agencies, brings medical information to members of the deaf community.
  • Uncompensated Care Drug Program
    • MHMMC Portland’s pharmacy provides free medications to qualifying cancer patients and discharged patients. Patients of the emergency department and MaineHealth Urgent Care Plus - Brighton Ave - Portland receive starter packs and some routine drugs at no charge when they are discharged.
  • Community Dental Health
    • MHMMC Portland provides dental services for oncology patients.
  • Homeless Health Partners/Preble Street Learning Collaborative
    • Homeless HealthPartners (HHP) is an MHMMC Portland initiative administered by CarePartners in collaboration with Preble Street and Portland Community Health Center.  HHP is comprised of one physical therapist and 2 Social Work Case Managers who outreach/engage homeless patients in short-term, targeted case management related to accessing primary care, care coordination and reduction of inappropriate ED utilization.
  • Homeless Clinic
    • Residents work in the homeless clinic supervised by attending physicians
  • Recuperative Care
    • MHMMC Portland, in partnership with Greater Portland Health (Federally Qualified Health Center) has opened a new recuperative care facility in FY22. With 15 beds, it provides high quality, patient centered health care that is accessible, affordable and culturally sensitive. That effort also helps to free up inpatient beds to allow other acute patients to be cared for in the hospital.

Workforce Training

  • MHMMC Portland provides a clinical setting for medical students from a number of medical schools as they rotate through the clinical services; post-graduate training in a number of specialties, in both residencies and fellowships; rural practice settings as part of resident education and as a service to the practices; and numerous sessions in which practicing physicians can keep their knowledge current.
  • Student Nursing Precepting
    • In FY23, 232 senior nursing students were placed with MHMMC Portland-employed RN staff as one-on-one preceptors. These placements culminated in 34,522 clinical hours of RN staff oversight of students in patient care settings throughout the organization. Oversight occurs during MHMMC Portland RN employee work time with full patient assignments
  • CNA Training Program
    • The CNA course is a state-approved, 190-hour course which is free of charge to students accepted to the program.  It includes classroom work, skills labs and clinical experiences.  The 11-week program is offered through the Center for Clinical & Professional Development with support and ongoing interaction from Human Resources.  Upon course completion, students sit for the state certification exam.  This initiative is a service to our community offering unique opportunities for diverse populations who otherwise might not be able to afford the education.  Many CNA graduates are ultimately employed by MHMMC Portland however, the program also provides graduates for other agencies or settings within the area.  CNAs are a valued part of the nursing team at MHMMC Portland.
  • Rehab Medicine Educational Program
    • The Rehab Medicine Educational Program allows students from NE area schools (e.g., UNE, UNH, USM) to learn from Rehab Medicine staff at no cost to them or their school.

Prevention and Education

  • Taxi Vouchers and Clothing
    • MMC MHMMC Portland provides taxi vouchers to patients who need transportation home, as well as petty cash and clothing for some patients.
  • Child Car Seat Program and Car Seat Safety
    • MHMMC Portland's Injury Prevention Education and Car Seat Specialist assists RNs and MDs with angle tolerance tests on infants </= 37 week, infants weighing </=2500g and any who need assistance with breathing or have underlying cardiac issues. The program loans car beds at no cost as well as car seats for special needs children. In addition, there are monthly community car seat inspections at the Falmouth Fire Department.
  • Tutoring
    • MHMMC Portland provides tutoring for students who miss school due to hospitalization.

Community Support

  • Climate Change Action
    • MHMMC Portland is in a unique position to address climate change by minimizing our carbon footprint and addressing the health effects linked to climate change by providing the highest quality preventative and acute care. MHMMC Portland has taken a stance on reducing the organization’s carbon footprint, and it is putting in place measures to help prepare the community for climate change and climate change induced natural disasters. 
    • A few examples of how MHMMC Portland invested in improving community health through environmental sustainability in 2022 include: LEED certification for the new Malone Family tower, electric car chargers, maintaining a smoke free campus, food and trash compositing, local food sourcing, and solar panel installations. In addition, the organization continues to conduct educational programs related to climate and health, the importance of clean air, the benefits of reusable resources, and the health implications of environmental exposure and how to manage exposure. MHMMC Portland recognizes its role in tackling climate change and has stepped up its efforts significantly in 2022.

The drug purchasing savings realized by the 340B program have directly benefited Knox County by allowing MaineHealth Pen Bay Hospital to stretch limited resources and provide additional benefits and services.  In FY23, the hospital provided $33,612,984 in uncompensated care for the community. The services we have been able to provide our service area includes:

Health Care Services

  • Financial assistance for prescription coverage for patients discharged from MaineHealth Pen Bay Hospital through our retail pharmacy
  • Assisted in providing indigent and charity care
  • Access to substance use disorder treatment and recovery initiatives
  • Depression screening workflow for adult and pediatric patients in primary & specialty care practice locations

Workforce Training

  • Free certified nursing assistant (CNA) program to help bolster our health care workforce
  • Student clinical placements for med students/residents, nursing students, CNA course students, radiology, PT/OT, NP/PA and other student clinical placements

Prevention and Education

  • Complimentary wheelchair van transportation for patients living within the service area
  • Complimentary taxi voucher program to support patients with transportation challenges post discharge
