The MaineHealth Waldo Hospital Patient & Family Advisory Council (PFAC) is an organization patients, families and care team members who are working to improve health care in our community. Council members collaborate with care team members and hospital leadership to champion patient safety and patient and family centered care.

Working together to make our communities the healthiest in America

The MaineHealth Waldo Hospital PFAC fosters collaboration between community members and the hospital in support of patient-centered care. An active PFAC is an important part of our participation in accreditation organizations such as the National Committee for Quality Assurance (NCQA) and its patient-centered medical home model. PFAC members are committed to working with the hospital to continuously improve patient and family experiences.

Council members collaborate with MaineHealth Waldo Hospital leadership and care team members to champion patient safety and patient and family centered care in alignment with hospital policies, planning, and ongoing processes for continuous improvement.

  • To collaborate with staff and to address current issues from a patient and family perspective
  • To work with staff to help ensure that patient and family perspectives and choices are acknowledged and respected
  • To gain and share patient and family input, establish a link between MaineHealth Waldo Hospital and the greater community and to identify patient needs
  • To work with staff to review and discuss quality improvement reports about ongoing efforts to improve health care practices and outcomes
  • To participate with staff in appropriate policy and program development, implementation and evaluation, in health care facility design, in professional education and in the delivery of innovative solutions
  • To gain a better understanding of the hospital's departments and services and to provide feedback
  • To create awareness of the PFAC’s role in the community and to encourage broad participation
  • To understand, provide feedback and/or establish action steps for goals set by MaineHealth that impact patient care

Current and former MaineHealth Waldo Hospital patients or patient family members are eligible to apply. Applications will be reviewed by staff and PFAC chairpersons or membership committee members. Once accepted, members sign a PFAC statement of confidentiality and will receive a PFAC new member packet with orientation material.

Members are expected to attend a majority of meetings per year. Meetings are held the fourth Monday of each month from 11:30 am - 1 pm in video conference rooms at the hospital. For more information, please call 207-301-8495.
