When a hospital and a physician are working together as one system, CMS designates the physician’s practice as a “provider-based” site of care. This means that any physician’s office that is part of a hospital or health care system must meet all quality standards required of hospitals ensuring our shared commitment to providing quality care is consistent. When a patient visits any MaineHealth medical center, hospital or practice; they are required to complete our unified consent to treat form once annually.
